Sanction for lineout throw not going 5m?

Other than under 9s , has anyone ever seen a lineout throw in so feeble it wasn't capable of traveling 5m ?

(basically I think that concept is a red herring)

Feeble? No. Greasy match ball slipping out the hands? Yep.

Utterly screwed up execution trying to do something clever and failing miserably? Guilty.

whatever the answer ... a FK in that position would always be taken as a scrum anyway so its rather a moot point wrt outcome.

I only ref age grade at present... I've not seen a single incident of a team opting for scrum.

I entirely agree that taking a scrum is common, but my point was that it does not happen 100% of time, so important to get the sanction correct.
