Where and what law dictates that 2 yellow cards result in a red? The IRB laws say that it is at the discretion of each Union or Match Organiser????

I believe it is law10.5b you are seeking.

[LAWS]10.5 Sanctions
(a) Any player who infringes any part of the Foul Play Law must be admonished, or cautioned and temporarily suspended for a period of ten minutes’ playing time, or sent-off.
(b) A player who has been cautioned and temporarily suspended who then commits a second cautionable offence within the Foul Play Law must be sent-off.[/LAWS]

You are looking at the wrong law. 10.6 merely states that you don't have to use an actual card to temporarily suspend or send off a player.
