Can a knock on occur from touch / outside the playing area?

Current Referee grade:
Level 1
As per 18.2.d:

The ball is not in touch or touch-in-goal if:
A player, who is in touch, kicks or knocks the ball, but does not hold it, provided it has not reached the plane of touch.

It does not dictate a specific direction of the knock or further clarify what happens should someone knock the ball forward from touch, in an effort to keep it from reaching the plane of touch.

Knock on or play on?...Thoughts?

If the ball crossed the plane, it is in touch, so play the lineout. If the ball never crossed the plane, it was never in touch, so play the knock on.

It's not in touch (whether or not the player is), so play continues as normal. It is knocked forward, so it's a knock on, scrum advantage to the opponent.
