trouble completing sending off form

Can anybody help me with completing a sending off form?
Does a head butt come under punch/strike or one of the other catergories?

A player must not strike an opponent with the fist or arm, including the elbow, shoulder, head or knee(s)

Looked on the RFU website from the disciplinary hearing from a couple of seasons ago with Joe Marler and Marcos Ayerza, with the headbutt, it is under punching/striking
