That's correct. Opp can only make changes at a pk or fk if it is their kick, or opposition do so.

How do you infer that? IRB makes no such distinction. If the ball is dead surely it is dead fior both teams.

In both cases outlined above, the ball is dead, and therefore the answer to
each question is in the affirmative.

Not if you want to go quickly - so if you want tap 'n' go PK, or QT then bin will have to wait.
